Establish a Healthcare Power of Attorney (or Medical Power of Attorney)

A healthcare power of attorney (HCPOA) also referred to as a Medical Power of Attorney, authorizes a person to act as an agent to make healthcare decisions on behalf of your loved one or senior should they be unable to act on their own due to illness or decline in cognitive function.

Should the primary POA be unavailable or unable to serve, a backup POA should also exist. 

Your senior’s trust documents typically include their healthcare POA. However, the POA forms are also available for download online at no cost and can be taken to a notary for proof of signature should your senior not have a trust.
